They absolutely can. There is no policy against it, and HC calls aren't vetted through Salt Lake. It's completely the stake president's call.

Bishop and stake president, on the other hand, are different. The FP would need to authorize that, and are very unlikely to. It would be a major exception.

It's possible. It would be case-by-case, though, and chances would be against it.

We reinstated from disfellowshipment a wonderful man in a previous ward. He had been inactive for at least ten years following his disfellowshipment, and his wife was my young daughter's primary teacher. He responded very well to our efforts to bring him back, and caught fire and hasn't looked back. I mentioned to the stake president that he would make a fantastic bishop, and he agreed, but said it probably wouldn't be approved. We (me and the stake president) made him our high priests group leader once he was restored to full fellowship.

Is there a note on someone's membership (after having their full blessings reinstated) if they have been disfellowshipped in the past?

No, but it would seem prudent for church headquarters to keep a record of past disciplinary actions.

Right. There isn't a notation that the average member (including local bishops and stake presidents) can see on membership records, but Salt Lake can see things we can't. I'm sure that all past things, like discipline and former major callings, are listed. The only time that an annotation would remain at the local level is if there is something in the background that leaders need to be aware of (abuse, financial misdeeds, etc. I had a brother in my old ward who was excommunicated for stealing tithing money as a counselor, and he obviously isn't supposed to have access to tithes and offerings again. So, that annotation will remain). 

I'm also sure that Salt Lake has internal notes on people who return to the Church after discipline/name removal for apostasy. I know people who have been rebaptized after something like that, but I don't think the church is going to take a chance on them being a bishop or stake president. That isn't going to show up on local records, though.
