It's hard to imagine rape causing enough physical damage to render a woman infertile without causing enough damage to kill her. An alternative would be that a sexually transmitted disease (especially gonorrhea, possibly chlamydia) could have been transmitted, resulting in pelvic inflammatory disease, which in severe cases can lead to scarring of the fallopian tubes, and subsequent increased risk for ectopic pregnancy or infertility. Life without effective antibiotics would have been incredibly rough.
