How does the First Presidency, under inspiration, call Temple Presidents and Patriarchs?

Friend of mine was recently and totally unexpectedly called to serve as a Temple President.

I've heard mission president candidates are provided by stake leaders and sent to Area Authorities who under inspiration recommend a few? to the First Presidency.

Not sure if existing Temple Presidencies offer suggestion to the Area Authorities who under inspiration recommend a few? to the First Presidency - or do Temple Presidencies recommend candidates directly to the First Presidency? 

Not sure about Patriarchs - as in I don't have any guesses as to who might recommend candidates, let alone to Area Authorities or to the First Presidency. 

If you dont know the SOP, I'm interested in hearing about your experiences with these types of callings and people.

Stake President recommends new Patriarchs through LCR. They are approved by Q12. Stake President then calls them, has them sustained, and sets them apart.  (Handbook 6.6.1)
